Thyroid cancer history is a contraindication for GLP-1 therapy. Your provider will review carefully.
MEN2 syndrome is a contraindication for GLP-1 therapy.
Pancreatitis history requires careful review before prescribing GLP-1 therapy.
GLP-1 medications are not recommended during pregnancy or breastfeeding.
Most insurance plans do not currently cover GLP-1s for weight loss. Branded medications are available at $499/month through manufacturer cash-pay programs for eligible patients.
Telehealth consent. I understand I am requesting an asynchronous telehealth consultation. My provider will review my submitted intake and make a clinical determination based on that information. I consent to this form of care.
Informed consent for GLP-1 therapy. I understand GLP-1 medications may cause side effects including nausea, vomiting, and GI issues. A prescription is not guaranteed and is at the sole discretion of my provider. I will not start, stop, or change any medication without provider guidance.
HIPAA authorization. I authorize SlimRx Miami and its affiliated providers to collect, use, and share my protected health information as necessary for my care. My information will not be sold or shared with third parties for marketing.
